After 30 days from the day of your judgment, come into our office with the following: a CERTIFIED copy of the Judgment Transcript from the District Justice Office, a Certificate of Residence completed, dated, and signed by Plaintiff or attorney, Notice of Entry of Judgment – two (2) and a stamped, addressed envelope for each defendant. Refer to Forms Available and Fee Schedule on our homepage for the current filing fee.

However, if you choose to represent yourself (pro se), you will be held to the same standards required for attorneys and must follow all local and state Rules and Procedure. You may come into our office or request it by mail. If you choose to request by mail the following information is required in your cover letter: docket number, plaintiff and defendants names and any additional information that may be helpful to us to locate your divorce decree. Include a Self-addressed-stamped envelope with your check or money order for processing. Refer to the Fee Schedule for the current Certification Fee.

Petitions for a name change (adults and minors) are filed in this Office and may be obtained at Adams County Law Library located on the 3rd floor of the Courthouse. Refer to the Fee Schedule for the current filing fee.
When you receive your bond, visit the Register & Recorders Office to file your bond then come to the Prothonotary Office to record your signature. Please refer to the Fee Schedule for the filing fee. Payment may be made by cash or check.
